State law requires Texas college students under the age of 22 to receive a vaccination or booster against bacterial meningitis (a quadrivalent or conjugate that covers strains A, C, W, and Y is required). The vaccine must be administered within the past five years and at least 10 days before the first day of classes.

Step 1: Check your shot record.
If you HAVE received the conjugate or quadrivalent bacterial meningitis vaccine within the last 5 years, proceed to Step 2 to submit your record now to Med+Proctor.

If you do not see the bacterial meningitis vaccine on your shot record contact your primary care provider or your local public health clinic to schedule an appointment to receive it as soon as possible.

If you have a notarized official Texas DSHS affidavit form, you may also submit this through Med+Proctor. Please visit the Texas Department of State Health Services website to learn more about immunization exemptions and how to request the exemption for reasons of consciousness affidavit.

Step 2: Create Your Account on Med+Proctor.

MSU uses the services of Med+Proctor to collect, review, and securely store all student documents pertaining to the meningitis vaccine.

1) Go to www.medproctor.com to create your account.
Be sure to use your Mustangs ID (including the capital M) as the student ID.

2) Complete the required workflow steps, please remember to confirm your email

address, and download any required immunization forms.

3) Follow all instructions, then upload completed forms back to your account.

For any questions regarding your Med+Proctor account you can email: help@medproctor.com or

Step 3: Approved!
Once your record is reviewed and approved by Med+Proctor, your Meningitis Immunization Hold will be removed from your MSU account. This process typically takes about 2-3 business days after record submission.
